There is amazing work being done across the country to help raise awareness about the importance of advance care planning and we sincerely hope to highlight this awesome national effort on the NHDD blog.  National Healthcare Decisions Day has been vibrant for years and with the 2011 online engagement initiative we have the opportunity to support each other's hard work and planning... but only if we share what we're up to! 

Please consider taking a minute to complete some of the information below to participate. One of our goals with the NHDD blog this year is to highlight what it is like to complete an advance directive. (So please remember you don't have to be a health expert to share your story)
